Am I eligible?

To purchase this plan you must be 18 years of age or older. Spouses are eligible for coverage as well as qualifying dependants from birth to age 26.


Is there waiting?

No. There is no waiting with Patriot Premier.


When will my discounts start?

Activation begins immediately and member materials are sent instantly via email when using a credit card during the application process. Activation takes eight days if you choose automatic bank draft activation as your payment option.


How do I find a participating dentist?

You can search for dentists that accept Patriot Premier through our Dentist Locator database. Find a Dentist


Does the plan have annual limits? How much will it pay?

No. There are no limits with Patriot Premier so use your card as often as you like.


Are there any extra fees?

There is a one-time $30 enrollment processing fee.


Is everyone eligible?

Everyone is accepted with the Patriot Premier plan.


What are my payment options?

Your initial payment and recurring (monthly) payments can be made by Visa, MasterCard, American Express, Discover and automatic bank draft.


Do the applicant name and the billing name need to be the same?

No. The applicant name and the billing name do not have to be the same.


Do I need to obtain proof of treatment?

No. You just show your card and get on-the-spot savings.


Is orthodontia (braces) included?

Yes. Orthodontia for children 16 and younger is offered at a significant savings. Members over age 16 are provided a 25% discount. Invisalign braces are 25% off of the usual and customary fee of the participating provider.


Is cosmetic dentistry covered?

Yes. Porcelain laminate veneers are listed on the fee schedule under “Restorative Procedures.” Chair side bleaching is available at a 25% discount off of the doctor’s usual and customary fee.


What if I want to cancel my plan?

You have 30 days from your start date (effective date) to provide written cancellation and receive a full refund of membership fees. Future billing is month to month. No refunds will be granted for partially unused months, but your plan can be cancelled at any time. Cancellations are acceptable via email, postal mail or by voice recording but should be at least one business day prior to your next automatic bill date to avoid being charged an additional month. Processing fees are non-refundable except in AR, FL, MD, ND, SC, SD, TN, VT.
